05f144eb19 Allow ArrayHandle to have a runtime selectable number of buffers
Previously, the number of buffers held by an `ArrayHandle` had to be
determined statically at compile time by the storage. Most of the time
this is fine. However, there are some exceptions where the number of
buffers need to be selected at runtime. For example, the
`ArrayHandleRecombineVec` does not specify the number of components it
uses, and it needed a hack where it stored buffers in the metadata of
another buffer, which is bad.

This change allows the number of buffers to vary at runtime (at least at
construction). The buffers were already managed in a `std::vector`. It
now no longer forces the vector to be a specific size.
`GetNumberOfBuffers` was removed from the `Storage`. Instead, if the
number of buffers was not specified at construction, an allocation of
size 0 is done to create default buffers.

The biggest change is to the interface of the storage object methods,
which now take `std::vector` instead of pointers to `Buffer` objects.
This adds a little hastle in having to copy subsets of this `vector`
when a storage object has multiple sub-arrays. But it does simplify some
of the templating.

5ef4e7eeee Make new style of ArrayHandle the expected style
What was previously declared as `ArrayHandleNewStyle` is now just the
implementation of `ArrayHandle`. The old implementation of `ArrayHandle`
has been moved to `ArrayHandleDeprecated`, and `ArrayHandle`s still
using this implementation must declare `VTKM_ARRAY_HANDLE_DEPRECATED` to
use it.

7811cc4b1e Add standard support for read-only storage
Many of the fancy `ArrayHandle`s are read-only and therefore connot
really create write portals. Likewise, many `ArrayHandle`s (both read-
only and read/write) have no way to resize themselves. In this case,
implementing the `CreateWritePortal` and `ResizeBuffers` methods in the
`Storage` class was troublesome. Mostly they just threw an exception,
but they also sometimes had to deal with cases where the behavior was
allowed.

To simplify code for developers, this introduces a pair of macros:
`VTKM_STORAGE_NO_RESIZE` and `VTKM_STORAGE_NO_WRITE_PORTAL`. These can
be declared in a `Storage` implementation when the storage has no viable
way to resize itself and create a write portal, respectively.

Having boilerplate code for these methods also helps work around
expected behavior for `ResizeBuffers`. `ResizeBuffers` should silently
work when resizing to the same size. Also `ResizeBuffers` should behave
well when resizing to 0 as that is what `ReleaseResources` does.

2f04dc3aef Simplify creating metadata in Buffer objects
Previously you had to create a special virtual object to place in the
`Buffer`'s metadata. This added a lot of difficulty that is probably
unnecessary.

Instead, just have `Buffer` hold an arbitrary object and some simple
functions to copy and delete it. There may be issues with type safety
across translation units, but we'll deal with that when/if we run into
it.

9b876df96f Remove PortalType from ArrayHandleImplicitTraits
The type for PortalType was declared before the class from which the
type came from. Normally this was not a big deal since the template was
resolved later, but nvcc seemed to have a problem with it.

813f5a422f Fixup custom portal iterator logic.
The convenience functions `ArrayPortalToIteratorBegin()` and
`ArrayPortalToIteratorEnd()` wouldn't detect specializations of
`ArrayPortalToIterators<PortalType>` since the specializations aren't
visible when the `Begin`/`End` functions are declared.

Since the CUDA iterators rely on a specialization, the convenience
functions would not compile on CUDA.

Now, instead of specializing `ArrayPortalToIterators` to provide custom
iterators for a particular portal, the portal may advertise custom
iterators by defining `IteratorType`, `GetIteratorBegin()`, and
`GetIteratorEnd()`. `ArrayPortalToIterators` will detect such portals
and automatically switch to using the specialized portals.

This eliminates the need for the specializations to be visible to the
convenience functions and allows them to be usable on CUDA.

33562561db Remove Set method from ArrayPortalImplicit.
All it did was abort, and defining this method meant that the
IsWritableArrayHandle check was incorrect for this handle type.

629271bceb Make sure all ArrayPortals have a Set method.
The current design for ArrayPortalVirtual makes it a requirement for all
array portals (that it wraps) to have Set defined. Thus, make sure Set is
defined for all ArrayPortal. Where Set is invalid, an assert is thrown if
something calls it at runtime.

fdaccc22db Remove exports for header-only functions/methods
Change the VTKM_CONT_EXPORT to VTKM_CONT. (Likewise for EXEC and
EXEC_CONT.) Remove the inline from these macros so that they can be
applied to everything, including implementations in a library.

Because inline is not declared in these modifies, you have to add the
keyword to functions and methods where the implementation is not inlined
in the class.

65c2261892 Add default constructors/destructors/assignment to ArrayHandle classes
The ArrayHandle classes all exclusively work in the control environment.
However, CUDA likes to add __device__ to constructors, destructors, and
assignment operators it automatically adds. This in turn causes warnings
about the __device__ function using host-only classes (like
boost::shared_ptr). Solve this problem by adding explicit methods for
all of these.

Implemented this by wrapping up all these default objects in a macro.
This also solved the problem of other constructors that are necessary
for array handles such as a constructor that takes the base array
handle.

7ff62d8d6b Explicitly add destructors and copy constructors to ArrayHandle classes
Under CUDA, the default constructors and destructors created are exported
as __host__ and __device__, which causes problems because they used a boost
pointer that only works on the host. The explicit copy constructors and
destructors do the same thing as the default ones except declared to only
work on the host.

This takes a reference to an array handle and fills it. This removes a lot of the pain of determining template arguments.

882c20a2cb Support DynamicArrayHandle::CastToArrayHandle for fancy ArrayHandles
Basically, this entails adding a constructor to each fancy ArrayHandle
that accepts the base ArrayHandle with the appropriate type and storage.

20c1a04894 CopyInto function for ArrayHandles
ArrayHandles in DAX have a CopyInto function which allows the user to copy an array handle's data into a compatible STL type iterator. Originally this was fairly straight forward to implement since array handles in DAX are templated on the DeviceAdapterTag. In contrast, VTKm array handles use a polymorphic ArrayHandleExecutionManager under the hood allowing a single array handle to interface with multiple devices at runtime. To achieve this virtual functions are used. This makes implementing the CopyInto function difficult since it is templated on the IteratorType and virtual functions cannot be templated.

To work around this, I've implemented a concrete templated CopyInto function in the class derived from ArrayHandleExecutionManagerBase. In the ArrayHandle class, CopyInto dynamically casts the base class into the derived class, then calls the CopyInto function defined in the derived class.

The drawback to this approach is that, should the user define their own class that inherits from ArrayHandleExectionManagerBase, they are not forced to implement the CopyInto function, unlike the other virtual functions.

6b68666d0c Created DynamicCellSet
The DynamicCellSet will be used in place of the pointer to a CellSet
in a DataSet. This will prevent us from having to cast it all the time
and also remove reliance on boost smart_ptr.
