- SourceCodeProcessor now consistently calls rule sets start / end
    if cache is not up to date
 - Both Mono and MultiThread Processors rely on PmdRunnable, just using
    different execution strategies
 - This also fixes https://sourceforge.net/p/pmd/bugs/1511/
